The SSCMS Foundation is an independent, 501(c)(3) non-profit organization that was created to support and enrich the middle school experience for all SSCMS students. Our mission is to expand and enhance the academic and extracurricular offerings at Sandy Springs Charter Middle School.

Required Forms for ALL PTO Foundation Sports

• PTO Foundation Sports Forms (ALL): Includes Application for Participation, Verification of Insurance, Student Athlete Conduct Agreement, Amateur Athletic Waiver and Release of Liability, Travel Release, Concussion Awareness & Cardiac Arrest Awareness Form
• SSCMS Pre-participation Physical Form: Must be filled out by a doctor within the past 12 months
